FOLLOW ASSESSMENT WITH PLAN FOR CHANGE…
1. Identify opportunities for improvement, listing them all in a consensus conference.
2. Set up action plans for just 2-3 opportunities (take small steps toward improvement). Action plans should be SMART:
Specific
Measurable
Achievable
Relevant to improved practice management and patient care
Time-oriented (maximum three months to see change)
3. Start a CQI cycle(Continuous Quality Improvement), using PDSA (Plan-Do-Study-Act), the scientific method for action-oriented learning:
Plan: the change to be tested or implemented
Do: carry out the test or change
Study: using before and after data, reflect on what was learned
Act: plan the next change cycle or full implementation
